Cataracts & Cataract Surgery
Cataracts are more common as you age, in diabetics, and in patients with glaucoma. Cataract surgery is the only way cataracts can be removed — they cannot be treated with glasses, contacts, or LASIK.
Cataract surgery is the most frequently performed surgery in the U.S., with nearly 4 million procedures each year. During surgery, your ophthalmologist removes the cloudy lens and replaces it with an artificial intraocular lens (IOL), restoring vision to between 20/20 and 20/40 for most patients.
Glaucoma
Glaucoma results from damage to the optic nerve fibers, often due to elevated eye pressure. Primary open-angle glaucoma (POAG) is the most common type and is painless — vision loss typically occurs gradually over many years.
Acute closed-angle glaucoma is a medical emergency causing sudden severe eye pain, headache, nausea, and blurred vision. Vision loss from glaucoma is irreversible, making early diagnosis and treatment critical.
Diabetic Retinopathy
Diabetic retinopathy is the most common diabetic eye disease and a leading cause of blindness in adults in the United States. It is caused by damage to the small blood vessels that nourish the retina.
In some patients, blood vessels swell and leak fluid into the macula causing blurry vision. In others, abnormal new blood vessels grow on the retina’s surface. Regular diabetic eye exams are essential for early detection.
Macular Degeneration
Dry AMD accounts for 90% of all cases with gradual central vision loss. Wet AMD — only 10% of cases — causes faster loss and accounts for 90% of legal blindness. Early detection through a comprehensive eye exam is the best defense.
Dry Eye Syndrome
Dry eye occurs when your eyes do not make enough tears or your tears are of poor quality. The condition is chronic but can be managed successfully with the right treatment plan.
Although dry eye is most prevalent in those aged 65 and older, our doctors are seeing increasing numbers of younger patients with dry eye symptoms — typically from prolonged use of screens, computers, and tablets.
Corneal Disease
The cornea is the clear, dome-shaped surface at the front of the eye that focuses light onto the retina. When it becomes cloudy, swollen, scarred, or misshapen from disease, injury, or infection, vision can become blurred, hazy, or distorted. Common causes include Fuchs’ dystrophy, keratoconus, infections, and trauma. Treatment ranges from medication and specialty contact lenses to corneal transplant surgery, depending on the severity and underlying cause.

Dr. Rafael Hernandez Jr.
Dr. Rafael Hernandez Jr.
Dr. Rafael Hernandez Jr. is a board certified ophthalmologist, fully trained internist, and the son of Dr. Rafael Hernandez, founder of Eye Consultants & Surgeons of Miami. He attended the University of Miami for his undergraduate Pre Med/Biology degree before earning his doctorate Cum Laude at the Autonomous School of Medicine of Central America in San José, Costa Rica.
He completed residencies in both Internal Medicine at St. Luke’s/Roosevelt Hospital (Columbia University, NY) and Ophthalmology at the Eye Foundation of Kansas City, University of Missouri. The father-son duo has worked together in private practice since 1996 with privileges at Mercy Hospital and Bascom Palmer Eye Institute.

Dr. Mario A. Sabates
Dr. Mario A. Sabates
Dr. Mario A. Sabates is a native of Camagüey, Cuba who arrived in the U.S. in 1967. He attended the University of Miami for his undergraduate degree, graduating Magna Cum Laude, and earned his Doctor of Medicine from the University of South Florida in 1986, graduating in the top quartile.
Dr. Sabates completed his Ophthalmology Residency at Tulane University (1988–1991), where he won the “Best Research Award” for his study on the effects of smoking on aqueous levels of Vitamin C. He then completed a Fellowship in Cornea, External Disease, and Refractive Surgery at Tulane University (1991–1992).
Dr. Sabates is the author of many peer-reviewed articles and book chapters, and has presented multiple papers at the International Society of Refractive Surgery (ISCRS).

Dr. Jorge L. Perez
Dr. Jorge L. Perez
Dr. Jorge L. Perez is a medical doctor specialized in the diagnosis, management, and treatment of diseases and surgeries of the eye. His special interests concentrate in corneal, refractive, glaucoma, and cataract surgery. His personal goal is to restore sight to those affected.
Dr. Perez completed 2 years of general surgery at Mt. Sinai Medical Center prior to completing a residency in ophthalmology at SUNY Downstate Medical Center under the supervision of Dr. Richard Troutman — a pioneer in refractive surgery and microsurgery of the eye.
Upon his return to Miami, he joined Dr. Rafael M. Hernandez in 1981. Together the practice has grown to include Drs. Mario A. Sabates, Rafael Hernandez Jr., and Armando M. Alegret, with over 100 combined years of experience and thousands of eye procedures performed.

Dr. Armando M. Alegret
Dr. Armando Alegret completed his undergraduate medical training in Camagüey, Cuba, obtaining a doctoral degree in medicine. After moving to the United States, he completed postgraduate residencies in Internal Medicine at the University of Miami and Ophthalmology at the University of Maryland.
He is a dual board-certified diplomat of both the American Board of Ophthalmology (ABO) and the American Board of Internal Medicine (ABIM). He has published over 20 peer-reviewed articles and presented research at multiple national and international specialty meetings.

Dr. Adiel G. Smith
Dr. Adiel G. Smith
Dr. Adiel G. Smith received his undergraduate degree from the Massachusetts Institute of Technology (MIT), where he majored in Mechanical Engineering with a specialty in medical device design. He then worked for four years at Johnson & Johnson, helping to manufacture the first-ever drug-eluting cardiac stent — an experience that motivated him to pursue medicine.
He received his M.D. from the University of Miami, conducting research at the Bascom Palmer Eye Institute throughout medical school. He then completed his ophthalmology residency at the prestigious Wills Eye Hospital — the oldest eye hospital in America — followed by a two-year fellowship in Vitreoretinal Surgery at the Cole Eye Institute of the Cleveland Clinic.
Dr. Smith has authored numerous peer-reviewed publications and served as sub-investigator on over a dozen clinical trials in macular degeneration, diabetes, and advanced imaging technology.
